How do I set up my first appointment?

Do I need to fill out forms?

Prior to  your first in-person visit, please real, fill and email the 5 pages listed under FORMS, or simply print and bring along with you.  Technically challenged?  No worries, we can handle it at time of service.

How long will my appointment be?

Your appointments will be approximately 60 minutes, unless otherwise noted. You will be seen on a one-to-one basis, by a highly trained manual therapist.

What should I wear to my appointment?

Please wear or bring with you comfortable clothing that you can move around in easily. Tee shirts, sweats, shorts and workout clothes are ideal. We do have the lovely “hospital gowns” if you prefer!

Do I need a doctor’s referral to come to physical therapy?

In most cases, a medical referral will be necessary for insurance reimbursement of physical therapy sessions. In the State of California, you may directly book PT evaluation and 12 follow up visits or 45 days (whichever comes first). After which, there needs to be a physician’s examination of the continuing symptoms, with a medical diagnosis, along with a signed therapy plan of care. This is not required for wellness based, rather than rehab based appointments. Wellness and maintenance therapy are not generally reimbursed by insurance.

How often will I need to come?

Your written doctor’s referral may recommend a certain frequency and duration for your treatment. However, it is ultimately up to the discretion of you and the therapist after your initial evaluation. Common frequency for this office is once a week.

How early do I need to book my follow-up appointments?

Susan is practicing reduced hours in different locations. At least 1 follow-up appointment can be booked at the end of your initial visit. Our priority is to spend quality time with our patients. In order to accomplish this, we schedule the appropriate time for each person, and do NOT overbook, overlap, or “squeeze-in” extra patients during your allotted time. Therefore, late arrival will result in a shortened treatment time, at the same agreed upon fee rate.

How much can I expect to pay?

Hourly rate is $150 in San Jose, $225-200 in Los Altos. Payable at time of service. We accept cash, checks, credit cards, Venmo and Zelle. Please see the Insurance FAQ’s for Insurance related questions and reimbursement.

Sick Policy

Because many of our patients have compromised immunity, you must be fever-free and diarrhea-free (without the use of medication to control these) for 24 hours prior to your scheduled appointment. We ask that you inform us of illness via phone, text (preferable) or email, and then recover at home. You will not be charged for this missed visit.
