Susan Casto Physical Therapy is a fee for service provider. We are not in network nor associated with any medical insurer, including MediCare.
What is Fee for Service?
Payment is made by the patient in full at the appointment time.
Why Fee for Service?
Insurance-based health care allows for treatment of a body part expressing a symptom. Frequently this body part is the result of a larger more widespread problem. This requires advanced training and time to understand the big picture and what is truly causing the pain. As fee for service providers, we treat patients for 1 hour for every visit in a private treatment room. Fee for service allows, you as the patient, the freedom of choice when it comes to your healthcare. You may choose to see providers outside of the network for quality care.
Will my insurance reimburse me?
Every insurance situation is different. Most will require a physician’s prescription for physical therapy. Most have various caps on the amount of therapy annually, some lumping occupational and speech therapy in the same category. Many will not reimburse for wellness care, maintenance or continued treatment to optimize your function. We can provide you with an invoice upon request, for your use in submitting for reimbursement from your insurance company. However, we cannot speculate on if or how much our out-of-network provision of care will be accepted, and then reimbursed , by your insurance provider. Please contact them directly prior to booking if that is a concern for you.
What can I expect to pay for each appointment?
Fees are based on time spent with you and the location of the service. The fee ranges are as follows:
- $225 for Initial Evaluation
- $200 for 60 minute appointments in the Los Altos office.
- $150 for appointments in San Jose residential studio.
- In-Home visits will depend on travel time plus $150/ approximately 45-minute appointment.
- $75.00 or the cost of the visit for cancellations with less than 24 hours notice (therapist’s discretion),
